2016 HIGH SCHOOL TEAM CAMP SHOOTOUT
Waiver and Release I approve of _____________________ participation in the SHU Women’s Basketball Clinics, and certify she is in good health and able to participate in camp activities. If medical attention is required for an injury during the camp, I accept responsibility for expenses and liabilities, and give my permission for such care. The camp or its site or staff does not provide medical or accident insurance for basketball camp participants. I agree on behalf of myself and my minor child(ren) to release and discharge the SHU Women’s Basketball Clinic staff, its officers, representatives and employees from any and all liability and claims arising out of or in any way connected with the basketball camp. The applicant hereby assumes all risks of injuries or damages to the person on behalf of myself, and minors to which I am a parent or guardian which might occur as a result of participation in the basketball camp. I agree to abide by the regulations for operation of the facilities used for the basketball camp and regulations for the registered individual activity. I further hereby agree to indemnify and hold harmless SHU Camp staff, its officers, representatives and employees from any and all liability that may occur to me or members of my immediate family in any basketball camp activity. This indemnification is to include and is not necessarily limited to any and all costs of litigation, medical expenses, and judgment or subrogation interests. We will consider your application for registration as acceptance of the above items. Dear Camper, I would like to personally invite you to our 2016 Seton Hill University High School Team Shootout. This camp is designed for high school teams interested in gaining competitive game experience as well while on a college campus. The camp will focus on game experiences, as well as getting to visit and play on a college campus. Please join us for a great way to improve your game, and more importantly HAVE FUN!!! Teams will be split into two four-
Camp fees: July 19 ($200 per team)___ Includes three games with PIAA certified officials
team ‘pools’ based on size of school, regular season scheduling conflicts, and any coaches requests
Checks payable to: Seton Hill University Women’s Basketball
that can be considered. PLEASE NOTE:
Teams will play four seven-minute each game with P.I.A.A rules.
High School coaches, be sure to contact Head Coach Mark Katarski with a complete roster AT LEAST one week prior to the camp.
WHAT TO BRING WITH YOU: Basketball shoes & protective gear (ankle braces, etc.) Water bottles Registration, payment, and waiver
In addition, ALL participating players MUST have a signed release PRIOR to participating in any events for the camp. (A sample of the release is included in the brochure)
