Applicants must be a returning Redondo Union High School (RUHS) student, transfer student or incoming freshman at RUHS for the 2017 - 2018 school year

Camps will be held at: RUHS, Hermosa Beach and Veterans Park
Cost:
Fees are $175 per camp, per athlete
All interested athletes need to complete the on-line application for registration: https://seahawkfoundation.8to18.com Application and Fees are due by 1st day of camp Schedule posted at www.seahawkfoundation.org District authorized coaches will teach all camps Fee and registration required NO refunds after first day of each camp
Camps are subject to cancellation, if they do not meet minimum number of participants Summer camps do not act as tryouts for athletic teams at RUHS Summer camps are designed only to develop skills and build foundations in a particular sport Summer camps do not constitute a guarantee of enrollment at RUHS for permit-seeking students (out of district)

2017 Sea Hawk Foundation Summer Sports Camps AUTHORIZATION TO PARTICIPATE AND RELEASE OF LIABILITY FORM 1.
I certify that information given on this application is accurate. If applicable, I have read and agree to abide by the California Interscholastic Federation (CIF) Student Athlete’s Code of Ethics, and all student policies and procedures established by the Redondo Beach Unified School District.
2.
I understand that many enrichment programs (including athletics) entail unknown and unanticipated risks, which could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I acknowledge that such risks simply cannot be eliminated without jeopardizing the essential qualities of these activities. As a participant in this program I acknowledge that the risks may involve but are not limited to: being struck by another participant, ball, etc., or may result in contusions, sprains, fractures, broken bones or concussions.
3.
I agree to provide proof of medical insurance with this authorization form. I agree and fully understand that the Sponsor (defined below) of the Summer Sports Camps will be liable only for any gross negligence on its part which occurs during authorized camp practices, classes, sessions, i.e., when the authorized instructor is present and supervising the enrichment event/activity.
4.
I hereby give my consent to my daughter/son to attend the RUHS Summer Sports Camp and enroll in the program(s) for which a selection has been made. I agree to ensure that my son/daughter has access to a parent or guardian (i.e., home, work or cell phone) in case of emergency, class cancellation, campus evacuation, or any other non-planned event.
5.
I understand and accept full academic and financial responsibility for selection(s) made on this application. The classes are not a prerequisite for my son/daughter participating in any program during the regular school year. There will be no grade or credit issued for the camps.
6.
I, the undersigned, hereby release and discharge the Redondo Beach Unified School District, Redondo Union High School and The Sea Hawk Foundation, including its officers, employees, agents, servants and volunteers (herein collectively referred to as the “Sponsor”), from all liability arising out of or in connection with the abovedescribed camp sessions, on behalf of or for the above-named minor. For the purposes of this agreement, “liability” means all claims, demands, losses, causes of action, suits or judgments of any and every kind on account of any injury to the person or property of the above-named minor that occurs during the abovedescribed camp sessions, and that results from any cause other than the gross negligence of the Sponsor.
7.
In the event of an accident, I give permission to render emergency aid and/or to take my son/daughter to a physician or medical facility. I hereby authorize the medical staff to take all necessary actions to treat my child and I agree to assume responsibility for the cost of all such services.
