ALL 2017-2018 PHYSICAL EXAMS MUST BE COMPLETED AFTER MAY 1, 2017 Waverly Health Care Urgent Care & Family Practice is now partnering with School District #145; $10 will be donated back to Waverly Activities Department for each Waverly High School/Middle School Sports Physical conducted between May 1 - August 4. *Physicals are not required for summer clinics. Baseline ImPact Testing Requirements Student-athletes who need to take the Baseline ImPact Test are the following: All 9-12 high school football players; any incoming 7th, 9th, 11th, or transfer students; or anyone who sustained a concussion over the 20162017 school year. Our athletic trainer will be holding sessions on May 22-24, at Waverly High School. Each student-athlete is required to reserve a spot at one of the sessions. Session times and sign-up instructions will be sent via email to parents prior to testing dates. NOTE - Baseline ImPact Testing is not required for participation in summer clinics and/or speed/weight training. The above requirements apply to student-athetes planning on participating in Middle School or High School athletics during the 2017-2018 school year.
