Carpinteria AYSO Beach Cup XXI Team Invitation Application Instructions Applications are now being accepted for entry into the Carpinteria AYSO Beach Cup The deadline to enter the tournament is March 5, 2016. Applications received by that date will be given priority for acceptance into the tournament; all others will be accepted based on available openings. Applications will be accepted to provide a narrow range of competition, based on a completed application. Incomplete applications will be returned. To be considered complete, your application must include all of the following: 1. 2.
Team Application Form, signed by the Team Coach and Regional Commissioner. An eASYO Tournament Team Roster report signed by your Regional Commissioner. RC designates are not acceptable. Roster Notes:
An eAYSO Tournament Roster is the only acceptable roster. Roster, as generated, must show the age appropriate coach training for the Coach and the Assistant Coach. Players should be listed in uniform order. Written alterations to the eAYSO roster are not acceptable.
Roster changes will be accepted at Team Check-in ONLY; Revised eAYSO Tournament Rosters must be signed by your Regional Commissioner.
Rosters must be comprised solely of players who were registered and played in the AYSO 2015 primary program. Eligibility exceptions that comply with National Policy 2.2a are acceptable.
Up to 3 guest players may be added to your team from another AYSO Region. The guest player’s Regional Commissioner must also sign the Guest Player form. The form must be included with your roster.

If accepted, your team commits to play the entire tournament. If your application is not accepted, you will be offered the opportunity to be placed on a waiting list, or if you prefer we will return your application to you within 48 hours of your decision. Refund: if you withdraw your application 30 or more days from the start of the tournament, a full refund will be issued. If you withdraw after that time, we will only issue a refund if a replacement team can be found, less any discount offered to register a replacement team. All information about the tournament can be obtained by visiting our website at www.carpsoccer.org. Please note that e-mail and the R683 website will be the primary means of communication for this tournament. We will be sending out information via email newsletter once your application is received. For “Badge Level”, insert R = Regional, I = Intermediate, A = Advanced, N = National. Also the date they were certified at that level.
In each box under “Center/Assistant/Boys/Girls”, provide the highest level they are competent and comfortable (e.g. U10, U12 etc.)
In “Player on Team”, indicate if the referee has a child who is playing in the tournament on this team.
Individual Referees must be listed. RRA will assign is not acceptable.

Referee changes are allowed and must be confirmed by email with the tournament in advance.
Referees may not provide credit to more than one team.
Scheduling requests for semi-finals and championship games will be the only scheduling requests considered. Referees who are parents with children on a team will not be scheduled during their child’s game.
