Deforest Area Youth Football League REFUND REQUEST FORM
Refund requests:
Pursuant to the Dane County Area Youth Football League (DCAYFL) Release Form, player registration fees will not be refunded after the final league weigh-in. In certain cases (e.g. injury, moved from area, other) the DAYFL Board will consider a refund request made after the last weigh-in and prior to the first game. These will be handled on a case by case basis. Final decisions on the amount of refund will be made by the DAYFL Board. If a refund request is made after July 1st the amount of the refund may be reduced by a $50 administration fee. Please complete the below form. Either email to a Board member or mail to DAYFL for consideration.
