Dear Applicant, Thank you for your interest in growing the game of tennis by providing an upgraded facility for those in your community. Please make sure that you meet the following criteria and initial by each requirement: _____The facility is a USTA member in good standing and agrees to hold USTA programs and/or tournaments as part of its on-going programming for a minimum of five years to develop new tennis players by providing clinics, either at no cost or at a nominal fee to the public. _____The existing facility will be available to the public and shall be primarily maintained by public funds. _____ Adopt-A-Court funding shall only be used for; repair, reconstruction and/or relocation of existing tennis courts, court equipment and/or fixed amenities. The improvements shall be substantially completed in the year the money is granted. _____The facility agrees, if requested, to disclose or provide information regarding all usage fees prior to grant assistance. _____The facility will comply with all applicable A.D.A. requirements. _____The facility will provide a safe, properly constructed and functional facility including other amenities (i.e. shade areas, water fountains, backboards, lighting, etc.) to make the game of tennis more enjoyable and to encourage the growth of tennis. _____The facility will provide photos and documentation of publicity after renovations, as well as information regarding new tennis programs, increased number of players annually for a period of five years after the completion of the upgrades. All applications for court surfacing/resurfacing must include a Deco Performa (please contact DecoTurf online at www.decoturf.com or 800-DECO-1ST. Funding will be granted only after completion of the Adopt-a-Court Application and determination by the USTA Adopt-A-Court Subcommittee that the facility complies with the stated criteria. Incomplete or revised applications will not be accepted. I have read and understand the above criteria and sign on behalf of the facility that we meet all requirements and will complete all obligations if awarded the Adopt-A-Court (AAC) Grant.
