The certificate number is usually stamped or written on the top right of the form. It is usually listed as the “State File Number.” If there is no state file number but a “Registered Number” is listed, index it in the Certificate Number field.
Type M (married or separated), S (single or never married), D (divorced), or W (widowed).
Informant: Type the name of the informant only, not the name of an official or clerk sometimes written in the field.
Include the city and state if they were written in the residence address field.
Type the name of the cemetery where the deceased was buried. Include the word “cemetery” if it was written in the cemetery field on the form.
In this case, you would index the cemetery as Local. (The location will be added later, using the residence information.)
When ampersands (&) and plus symbols (+) were written as part of the funeral home's name, index these symbols as “And.” For example “Taylor & Son” would be indexed as Taylor And Son.
