2015 – FREE BEGINNERS LACROSSE INTRO CLINICE Sessions are held at Bernard Campbell Middle School, 1201 NE Colbern Road, LSMO 64086 (please park in Campbell Middle School parking lot, NOT in the neighborhood behind the school)

B) Should immediately advise his or her coach or supervisor of such condition(s) and refuse to participate. Acknowledges and fully understands that each Participant will be engaging in activities and may travel that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inaction or negligence, but the action, inactions or negligence, but the action, inaction or negligence of others, the rules play or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time. Assumes all the foregoing risks and accepts personal responsibility for the damages following such injury, permanent disability or death. Acknowledges and fully understands that the LEE'S SUMMIT YOUTH LACROSSE LEAGUE does not provide nor supervise transportation for the minor participant to any team events, including but not limited to practice, games in-town, games or tournaments out-of-town and further acknowledges and fully understands that the responsibility for said transportation is that of the parents and/or legal guardian of each minor participant, regardless of whether or not said transportation is being provided by a fellow teammate or person associated with the team. Releases, waives, discharges and covenants not to be sure the LEE'S SUMMIT YOUTH LACROSSE LEAGUE, its affiliated clubs, their respective administrators, directors, board members, agents, coaches and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as "releases", from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be used in whole or in part by the negligence of the releases or otherwise. We have read the above waiver and release, understand that we have given up substantial rights by signing it and sign it voluntarily.
