INDIANA STATE UNIVERSITY DEPARTMENT OF ATHLETICS NON-RENEWAL OR REDUCTION DECLARATION Coaches: Please complete and submit this form with the names of the student athletes that are on any athletic scholarship during the current academic year who will NOT receive the same amount of athletically-related financial aid next academic year that they received the prior academic year for any reason, including graduation, transfer, reductions, or any other reason. This form must be submitted to the compliance office as soon as possible, but no later than June 15th. Upon receipt, the ISU Financial Aid Office shall be notified and shall mail the mandatory notices to the student athletes per NCAA Bylaw 15.3.5.1. SPORT
