Completed fact sheet listing involvement in school and parish through which he/she has made a positive contribution and any community activities such as participation in civic organizations and part-time jobs. *Please feel free to use your own format for Fact Sheet while ensuring all information is included. 2. Two-page essay explaining how his/her involvement in a CYO, youth ministry program or community service activity has influenced his/her outlook on the future. Special emphasis should be placed on Parish involvement. The committee will also consider any special circumstances relating to personal or family situations (use additional sheets as necessary). 3. Two letters of recommendations (family members not acceptable; Pastor recommended). 4. High school picture (copy accepted) for press release. 5. Recipients (amounts vary) will be notified by mail.
