Locomotion 2018 TOURNAMENT INFO Dear Coach or Team representative. These are issues that come up each year at check in, please read.
1. All official rosters require a RC “wet signature”, all applications must have been electronically approved by RC. Rosters will be finalized at team check in, the most current roster available at check in is the roster you must use for the tournament. This is a roster with original signature from your RC, if you have too many players on your roster you must choose who to cross off the roster and any crossed off will not be allowed to participate.

If you have doubts as to whether you submitted a roster, bring a new one with an original signature. Have your rosters in jersey order ALL ROSTERS MUST BE SOME FORM OF OFFICIAL AYSO ROSTER . HAND WRITTEN INFORMATION ON ROSTERS IS NOT ACCEPTABLE You must be prepared to show a current player registration form (white form, consent form, player release form, these are terms that are often used for this form.) This is the form that would have been submitted when all players registered to participate in AYSO. Per National guidelines every player registration form is required to have “E-signature” OR a “wet” signature from parent or guardian, forms lacking them will not be allowed to participate. Bring a copy of every players form to the team check in. Coach and player ID badges are not required to be presented at check in, however if they are not completed properly when your team checks in for their first game you will not be allowed to participate. We suggest having them done by check in for inspection. You will be mailed 6 official Locomotion game cards, these are the only game cards we accept. If you come to the check in with something other than the provided game cards you will be required to step out of line and fill in 6 new ones. All game cards MUST be filled in, in jersey order. We go to extra time and expense to provide these official cards, the printing does not run if they get wet. Fill in the game cards with a ball point pen (the ink doesn’t run) If you make the decision to fill in the cards with an ink that we determine will run if it gets wet you will be asked to step out of line and fill them in again with an acceptable ink. Even if your team is at a tournament or some other team function on the day to check in for Locomotion you MUST have a copy of the E-signature or original wet signature medical release/registration forms. Make an additional set of copies, have them signed and bring them to the check in, we must see them.
THE GAME CARDS WE PROVIDE ARE NOT CHEAP AND THE INSTRUCTIONS ARE CLEAR, IF THEY ARE NOT FILLED IN PROPERLY YOU MAY BE SUBJECT TO A $5.00 FEE FOR A NEW SET OF CARDS
