Revolution Prep Company Cell Phone Agreement

This Company Cell Phone Policy is being entered into effective upon employment with Revolution Prep, LLC (“Company”). 1. Company cell phones are issued to the Employee for business use only. 2. The Employee will refrain from any texting (personal or business-related). 3. The Employee will be 100% financially responsible for any charges incurred for: a. Any personal calls made or received b. Any text messages sent or received * c. Any 411 or Directory/Information Assistance calls 4. If the Company cell phone is misplaced or lost, the employee will immediately notify the Company, will be 100% financially responsible and will be required to replace it. 5. If the employee has a Company iPhone, the employee must expense a hard cover case amounting up to $30. 6. If the Employee at any point in time discontinues working for the Company, the Employee will be responsible for returning the phone, charger, and hard case (if applicable) within three days.
