NCAA Bylaw 16.5.2 allows an institution to provide permissible room and board to student-athletes when they are required to stay on campus during institutional breaks for competition and practice. Please refer to this bylaw when completing this form. This form must be completed and approved by the Compliance Office prior to any break/vacation per diems being disbursed to SAs.

LODGING Lodging may be provided for each day of the vacation period when Dorms are not open. The reimbursable rate is $12 per day. List Dates for Lodging Per Diem:

SPECIAL NOTES Please submit a roster of all Student-Athletes who you are requesting to receive this per diem. If a student-athlete is living at home during the institutional vacation period, meals and lodging expenses may not be provided An institution may provide a 4th meal (for nutritional purposes) during vacation periods, provided the SAs are required to stay on campus for practice and competition. The meal may not be replaced with a per diem. (indicated as E above). Please submit with the reconciliation paperwork: Receipts for meals provided (not associated with team travel), a copy of this form, and a per diem form with signatures from each student-athlete who received a per diem/housing allowance per the bylaw.
