OFSL Chapter & Inter/national Roster Submission

 The OFSL Chapter Roster Submission is needed to review that all membership information is updated for the beginning of each semester.  Roster Review Meetings are to be held prior to the last day of classes by 5PM and meetings are to be scheduled fifteen (15) business days prior to the last day of classes.  The End of Semester Paperwork serves as a method of tracking changes in chapter leadership as well as questions about membership and programming.  Standards of Fraternal Excellence Submissions are used to track and asses the important aspects of chapter programming and operations in order to evaluate if the chapter meets minimum expectations for fraternal organizations.

 The Membership Intake Intent Form serves to find out if chapters of UGC and NPHC organizations will be conducting membership intake for the semester.

Schedule and Attend Membership Intake Review Meeting with Council Advisor

Ten (10) business days prior to the start of Membership Intake Process

 The purpose of this meeting is to review the Membership Intake Process outlined for the semester. In order for this meeting to occur the Chapter President and Membership Intake Coordinator must attend and have completed:  Membership Intake Registration Form (MI2)  Any Intake Paperwork to be signed by the Council Advisor for the Inter/National Organization

Five (5) business days following Informational Meetings

Interest Group Activity Report (MI3-B)

Five (5) business days following Interest Group Activity

 This attendance list must be submitted even if the chapter is NOT conducting Membership Intake for the semester.  ***This is only completed if your organization has an

interest group process.*** Please make sure to submit this form after any and all interest group activities!!!

Any and All changes to the Membership Intake Registration Form (MI2) Inter/National Organization Approval Form (MI4)

 Must be signed by an inter/national or regional representative.

Verification of Aspirants (MI5) Yellow Cards for all Aspirants Aspirant Disaffiliation Deadline

 The period where an aspirant can be removed from the OFSL Chapter Roster and not be included in the Chapter Grade Report at the end of the semester.

End of Intake Report (MI6) Neophyte Presentation Agreement (MI7-A)

 Must be accompanied by the Marshall Student Center EMS Approval Form.

 The period where a new member can be removed from the OFSL Chapter Roster and not be included in the Chapter Grade Report at the end of the semester.

Twenty-five (25) business days by 5PM EST if the event is more than 250 miles away from the University or overnight. Ten (10) business days prior to the event by 5PM EST. Fifteen (15) business days by 5PM EST if the event is more than 250 miles away from the University or overnight.
