Please see the information that follows for procedures and guidelines associated with the nomination process. All are welcome to submit a nomination! If your candidate meets the requirements on the following pages, they will be considered for that year’s induction class. Please note that applications are due each year by February 29, 2016, in order to provide ample time for consideration. Completed nomination forms should be returned to: Dale Krzynowek Garfield Heights High School 4900 Turney Road Garfield Heights, Ohio 44125 Or emailed to [email protected] When completing your nomination form, do not assume that the Hall of Fame Selection Committee knows the individual you are nominating. Your form must not only introduce your candidate to the Committee, but also, thoroughly explain why he or she deserves this honor. The Hall of Fame Selection Committee will select the finalists. Voting on those finalists for induction will be done by the Hall of Fame Selection Committee and two alumni. Any nominee not gaining induction into the Hall of Fame will automatically be a candidate for the next three years. Re-nomination is not necessary during this time period; however, you may update your nomination form if appropriate. Considerations for Nominations
Garfield Heights High School graduates who enthusiastically participated in athletics;
Garfield Heights High School graduates who excelled in sports at the collegiate, amateur, or professional level;
Garfield Heights City School District Administrators, Coaches, or District Leaders who positively contributed to Garfield Heights High School Athletics;
Garfield Heights City School Booster and/or citizens who have had a major influence or eagerly made contributions to Garfield Heights Athletics;
Garfield Heights High School graduates must have been out of school or active service for at least five years;
Garfield Heights High School graduates must have earned a varsity letter and must have attended Garfield Heights High School for at least two years. Additionally, you may wish to note any “All-League” honors or recognitions;
Garfield Heights City School District non-athletes who have distinguished themselves by demonstrating leadership, dedication and outstanding service to the athletic program for a period of at least five years.
