PARENT/GUARDIAN PARTICPATION AGREEMENT/CODE OF CONDUCT One of the primary objectives of the Cal Youth Sports Program is to instill in our athletes the principles of good sportsmanship, honesty, courage, and respect for themselves, authority and each other. To achieve this, Cal Youth Sports will not tolerate actions of players, parents, coaches or spectators that are detrimental to our Club in any way. Any person(s) found in violation of the following may; lose the right to attend Cal Youth Sports events and/or be dismissed from the Cal Youth Sports Program. As Parent/Guardian, I/WE agree to adhere to the following: 1. Registration fees are non-refundable after July 1, 2015. Your child will not be placed on a team unless his or her registration fee has been paid in full. A $35.00 processing fee will be assessed on all returned checks. 2. There is absolutely NO REFUND issued for any reason; this includes cases involving injury, illness, moving, or quitting. NO EXCEPTIONS!! 3. Parents will be held financially responsible for all equipment issued to their children. Replacement cost can exceed $200.00. The equipment issued is the sole property of Cal Youth Sports. If the equipment has not been returned by January 31, 2016. Cal Youth Sports reserves the right to report the equipment stolen and prosecute to the fullest extent of the law. 4. Furnish valid proof of Birth of applicant to Cal Youth Sports. (i.e.: Birth Certificate, Passport or ID) 5. Abide by doctor’s decision in all matters of player’s health and injuries and physical ability to play. 6. Support your coaches and refrain from excessive “sideline coaching”. 7. Support the coaches, players and officials by help teaching the value of commitment to the teams, sportsmanship, ethical conduct and fair play. 8. Accept the decisions of officials on the field as being fair and called to the best ability of the officials. 9. Support your child and give constructive criticism in a private moment, never in front of other parents, player, coaches, officials or spectators. 10. During practices and games no parent or unauthorized person will be allowed on the field. You may watch from designated areas only. 11. Refrain from using profanity and remain under control in order to set a good example for players and other spectators. 12. Parents/Guardian will not criticize other parents, players, spectators, or coaches (from either team) by words or gestures. 13. Parents/Guardian will not allow their child or guest to engage in any inappropriate or unsportsmanlike conduct with other parents, players, spectators or coaches (from either team). Inappropriate behavior is cause for immediate ejection from the stands.

Parents/Guardian will not allow their child or guest to engage in any inappropriate or unsportsmanlike conduct with other parents, players, spectators or coaches (from either team). Inappropriate behavior is cause for immediate ejection from the stands. 15. Parents/Guardian or guest are not allowed to interrupt practices or games expecting to talk with the coaching staff. All concerns must be addressed to the Team AD/Head AD (who will approach the staff in the event of a concern requiring immediate attention). All conversations with the coaching staff are to be reserved for after practices or games (at the coaches’ discretion). 16. Follow the proper Chain of Command when filing a complaint or voicing your opinion regarding any possible rule infraction or concern within Cal Youth Sports. The Chain Of Command will be as follows: You may only advance to the next level in the chain of command providing your situation has not been handled within 72 hours from your initial filing and you need further assistants. 1. The Team AD 2. Chapter AD 3. Head Coach  If your complaint is regarding the Head Coach you may go directly to the League President/Cheer Director. (Which-ever is applicable) 4. The President or Cheer Director of Cal Youth Sports. (Which-ever is applicable) 5. A letter written to the Southern California Conference AAU. If you go directly to the Southern California Conference AAU without permission, you may be removed from this Chapter WITHOUT the right to appeal. 17. Alcohol, tobacco, or illegal drugs are strictly prohibited in practices, games and any event areas. 18. I understand that I am responsible for my guest(s) violating any of these rules. I understand that the actions of my guest(s) could cause me and my child to be removed from the practice, games, and/or the program. 19. Cal Youth Sports is not a babysitting facility and each parent/guardian is required to pick up their child/children promptly once practice/game has been completed.
