Cancellation Policy: onPeak, the SIGGRAPH 2011 official housing partner, will acknowledge your hotel reservations by email, fax, or by postal service (if no email or fax number is provided). Your acknowledgement will indicate the name, address, telephone number, deposit information and confirmed room rate for your hotel. If you need to make any changes or cancel your reservation on or before Thursday, 28 July, contact onPeak. Beginning Friday, 29 July 2011 changes or cancellations must be made directly with your assigned hotel. Payment Guarantee: A one-night room and tax deposit (CAD) is non-refundable after 21 July. The non-refundable deposit does not hold your reservation if you do not check in on your confirmed arrival date. SIGGRAPH 2011’s deposit policy supersedes any individual hotel’s deposit policy. If you are guaranteeing your reservation by check, it must accompany this form and be received no later than 15 July 2011. Credit card information is required at time of booking to hold the reservation for all deposits paid with check until the check is received. Please fill out the credit card section and add one night’s room and tax (CAD) (12% HST + 1.5% DMF) per room, subject to change. Shuttle Bus Offered: Attendees who use the SIGGRAPH 2011 hotel reservation system to make reservations at hotels served by the SIGGRAPH 2011 shuttle buses will receive a shuttle wristband when they check in. Attendees who do not book through the SIGGRAPH 2011 reservation system and wish to use the convention center shuttle can purchase wristbands at the SIGGRAPH 2011 Store.
**Important! Groups of 30 or more. You must read, sign, and date below. Otherwise rooms will not be confirmed.** On or before 17 June, 2010, Group may release up to 10% of the total room nights contracted without penalty. After 17 June, 2011 should the actual room nights utilized on a per night basis fall below 90% of the rooms contracted as of that date Group agrees to be financially responsible for those unused room nights (calculated as number of rooms below 90% times the contracted hotel room rates). Should Group cancel the contracted room block at any point after signature, SIGGRAPH 2011 reserves the right to hold Group responsible for the total number of contracted room nights multiplied by the contracted hotel room rates. Cancellation is defined as the total non-use of the contracted room block. Any changes to a contracted room block and all group cancellations must be received in writing by the SIGGRAPH 2011 Event Manager. Any payment due must be received no later than thirty (30) days after date of invoice. Important Note: Should a Group wish to block 10% more room nights than its previous actualized block, additional rooms may be subject to a non-refundable deposit.
