Terms & Conditions By signing below, I/we, the parent(s) / guardian(s) of ______________________________________ (players name) 1. Endorse my/our child’s participation in the Baden Youth Association (BYA) program. I understand the primary objective of this program is to provide baseball training and guidance for the children of our community. All participants, coaches, parents and spectators are to abide by the BYA Code of Conduct (as published) at all times during the season. 2. Agree to release, absolve, indemnify and hold harmless the Baden Youth Association, its sponsors, officers, managers and coaches in case of injury to my/our child during these activities and when being transported to/from these activities. Transportation to and from BYA activities is at the expense, responsibility and liability of the participant. 3. Understand that the insurance carried by the BYA covers only the amount that is not provided by my/our insurance carrier. 4. Give my/our consent for the BYA representative, coaches, or staff to procure emergency medical treatment for my/our child in case of a medical emergency. I understand that I am responsible for any medical charges incurred and that the BYA carries Liability Insurance and Secondary Medical Coverage only. 5. Agree to inform the manager (and coaches) of my/our child’s team of any known medical condition(s) or allergies pertinent to my/our child and suggest our preferred course of action should it become necessary to respond while under the supervision of the manager (or coach) and supply an emergency phone number that will answer. 6. Grant the BYA my permission to use my or my child’s photograph publicly to promote the association. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. 7. Are able to furnish, upon request by the BYA, a certified copy of the birth certificate of the above-named child. BYA Code of Conduct I hereby pledge to provide positive support, care and encouragement for all players, coaches, other parents and umpires in the Baden Youth Association: • • • • • •

I agree to cheer, let the coaches coach, let the umpires ump and let the players play. I agree to respect the schedules of the coaches and teammates by having my ballplayer arrive on time for practices and games and will notify my coach if my child will arrive late or not able to attend. I agree to be a positive role model and will not engage in public displays of anger. I will promote good sportsmanship by respecting opposing fans, coaches, participants and umpires. I agree to respect coaching decisions regarding playing time, position and placement and will refrain from coaching any player while a game is in session. I will not approach players or coaches before, during or immediately after games with anything other than support. All questions/concerns regarding players or coaches will be addressed the day after the game in an atmosphere conducive to problem solving. I agree to file, in writing to the League Board of Directors complaints regarding: violation of rules, questionable conduct such as abusive behavior by a player, coach or fan and irreconcilable personality conflicts.
My signature verifies that I have read, understand and agree to abide by this Code of Conduct. I also agree to the consequences determined by the Baden Youth Association.
