PARENT/GUARDIAN CONSENT: I (we) am (are) the parent(s) or legal guardian(s) of the player (and the player) submitting this registration. I (we) consent and agree that the player may participate in any and all Nationals tryouts, and if selected and signed to a team, participate as a part of the Nationals soccer club. In consideration of the recreation and instruction being furnished and acknowledging that soccer is a contact sport, I (we) on behalf of the player (and the player), hereby agree to release, hold harmless, defend, and indemnify Nationals, and its owners, shareholders, officers, members, agents, assigns, managers, coaches, staff, volunteers and affiliates, and any and all communities and soccer facilities where Nationals programs are held from any claims, actions, proceedings, debts, liabilities, agreements, controversies, promises, obligations, damages, costs, expenses, attorney’s fees and demands in tort, contract or otherwise arising out of or related to the participation, practice, and play in the tryouts or the program whether known or unknown, accrued or not accrued. We further certify that the player is not currently registered or signed by any team in any other non-school league affiliated with Michigan Youth Soccer Association (MSYSA), Michigan Youth Soccer League (MYSL), Mid-Michigan Youth Soccer League (MMYSL), Western Suburban Soccer League (WSSL), Michigan State Developmental Soccer League (MSDSL), Michigan State Premier Soccer League (MSPSP), Directors Academy (DA), Midwest Regional League (MRL), National League (NL), Boys Club Championship (BCC), United States Specialty Sports Association (USSSA), US Club Soccer or any other league which participates in any of these leagues or the Michigan State Youth Soccer Association (MSYSA).
