Ball State University Change of Status Policy & Procedure Athletics Compliance Office NCAA Bylaw 15.5.11.1 Eligibility Requirement. To be eligible to represent an institution in intercollegiate athletics competition, a student-‐athlete shall be included on the institution’s squad-‐list form. NCAA Bylaw 15.5.11.2 Squad List Form. The member institution’s athletics director shall compile a list, on a form maintained by the Awards, Benefits, Expenses and Financial Aid Cabinet and approved by the Legislative Council, of the squad members in each sport on the first day of competition and shall indicate thereon the status of each member in the categories listed. NCAA Bylaw 15.5.11.2.1 Procedures. The following procedures shall be used in regard to the squad list required in Bylaw 15.5.9.2: (a) The form shall be available for examination upon request by an authorized representative of another member institution, the NCAA, and, if the institution is a member of a conference, an authorized representative of the conference; (b) A supplementary form may be filed to add names of persons not initially on the squad or to indicate a change of status; (c) A student-‐athlete’s name must be on the official institutional form to qualify to represent the institution in intercollegiate athletics; (d) The athletics director (or his or her designee, who may not be a coaching staff member) shall sign the form for each sport. The head coach in each sport shall sign the form for the applicable sport. NCAA Bylaw 15.5.11.3 Drug-‐Testing Consent-‐Form Requirement. Any student-‐athlete who signs a drug-‐testing consent form must be included on the institution’s squad list form, and any student-‐athlete who is included on the squad-‐list form must have signed a drug-‐testing consent form pursuant to Bylaw 12.7.3.

All student-‐athletes must appear on the Squad List before the first competition to be eligible to compete. 2. Coaches must submit a Change of Status form for any student-‐athlete either added or deleted from the Squad List to the Athletics Compliance Office. Procedure: 1. The Athletics Compliance Office will keep all Change of Status forms on file. 2. The Change of Status form will be processed and an updated roster for the appropriate squad will be distributed to the Head Coach, Financial Aid Office, Sport Supervisor, Faculty Athletics Representative, Equipment Office, Athletic Training Staff, Strength & Conditioning Staff, Student-‐Athlete Advising Office, Registrar Office and Media Relations Office. 3. The Registrar Office will produce an updated Squad List and Eligibility Checklist for approval by the Athletics Director (or designee), Faculty Athletics Representative, Athletics Compliance Office and Head Coach. 4. The Change of Status form must be completed within one (1) week of the status change for those who have been deleted from the squad. 5.
