**New and Transfer students must also complete the following: 1) Baseline Concussion Test before you can compete. 2) Verification of Sickle Cell Trait Status on athletic physical form Email [email protected] to schedule appointment for baseline testing which takes approx. 20-25 min.
Please fill out Allergies and Medications (Prescriptions) under the “Medical” tab of the “My Info” section of your athlete profile.
**MUST SUBMIT a copy of the FRONT and BACK of your insurance card, either uploaded or in person. **If you DO NOT have primary insurance, please type NONE in the Company Name.
HOW TO SIGN FORMS **Must print out the “Annual Physical Form” and have it filled out and SIGNED AND STAMPED by a medical doctor or the Baruch College Student Health Center. **The two “Read Only” forms just need to be opened and read. It tracks if you have opened or not.
