Please return this completed registration form to the Auxiliary Programs Office. All families who plan to attend Extended Day should fill out a new Registration Form each trimester. Extended Day offers: • A customized program calendar to meet your family’s needs. This Extended Day Program Calendar lists every day that the program will be in operation. Use the form to mark the days that you would like to sign up for the program. • Full Day: 3:00 – 6:00 p.m., or Half Day: 3:00- 4:30 p.m. or 4:30 – 6:00 p.m. (following a club) To sign up: Step 1: Fill out the Trimester 1 Extended Day Program Calendar. Choose half days or full days. Step 2: Fill out the registration information below. Add up the total number of half days and/or full days. Step 3: Drop-off your completed form to the Auxiliary Programs Office. Please note: • If you need to switch your reserved days from your Program Calendar, please email us at least 24 hours in advance, or you will be charged at the drop-in rate. • If you attend Extended Day on a day that is not reserved, you will be charged at the drop-in rate. • No portion of the reserved but unused days will be refunded, prorated, or applied to future trimesters.
