GUYANA VISA REQUIREMENTS AND INSTRUCTIONS READ CAREFULLY: Incomplete or delayed submission of the visa application can result in you missing your program! To apply for your visa: Download and print the visa application at: http://www.guyana.org/govt/visa_application.html Print out the 2 page application. Then, click on “Next page” at the bottom of the application and print out the signature page. Please note that there are 3 pages total. There are 2 application pages and one signature page. You need to print out all of them. Please complete the applications following the attached instructions and using the sample filled-out form as a guide. If you make a mistake on your visa application, please fill out a new one. The embassy will not accept visa applications that contain information that has been whited out or crossed out. You must submit 3 completed visa applications. This includes 2 application pages and 1 signature page in each completed application. Paperclip 3 - 2”x 2” photos (in addition to the 2 photos needed for your passport application) to your visa application. On the back of all your photos, please print your name. Mail your passport application, visa application, and passport photos to the address provided below. You need to use a mailing service to send us your travel documents (i.e. DHL, UPS, or Fed Ex). Please remember to retain your tracking number.
