I would like take opportunity to introduce the executive committee of Jersey United Soccer Club Spartans: Martin Brown is the President and CEO Dennis Jackson is the club’s Director of Coaching Please find attached several forms that must be completed and mailed back to us along with a copy of your player’s birth certificate and 2 passport sized photographs (please write your player’s name on the back of the photographs). Please mail and/or email back: Club Contract NJ Medical form NJ Player Registration Form US Club Soccer Form JU Parent Code of Contact Birth Certificate 2 passport sized pictures Please note no player will be carded until these documents are returned. Also attached is various information on the club, tournament and staff information will be presented to you at the annual season welcome meeting.
