I certify that my child is physically and mentally able to participate in te Activity and that my child will follow all appropriate safety precautions and instructions. I am aware that ECU will not be liable for injuries that may occur during participation. I therefore agree to assume all risks and responsibilities in any way associated with this Activity. In consideration of and return for services, facilities, and other assistance provided to me by ECU in this Activity, I release ECU, its governing board, officers, agents, employees, and other students acting as employees, from and against any and all liability, claims. Demands, and actions caused entirely or in part by any acts or failures to act of ECU, including but not limited to negligence, mistake, or failure to supervise by ECU. I acknowledge that ECU does not carry life, health, or accident insurance on my
