TOGO VISA REQUIREMENTS AND INSTRUCTIONS READ CAREFULLY: Incomplete or delayed submission of the visa application can result in you missing your program! To apply for your visas:
Access electronically fillable visa application at: http://www.traveldocs.com/uploads/forms/Togo_VisaDC.pdf
Print your full name and country of service legibly on the back of your passport photos.
Do not use white out or cross anything out on the visa form. If you make a mistake, you must complete a new visa application. The embassy will not accept any visa applications with any information that has been whited out or crossed out.
Please type the application following the instructions below and referencing the example provided in this document. Before printing confirm: Your surname is your last name Please note dates as Day-Month-Year. For example: 07-Sept-1985. Be absolutely sure you include both place and date of birth Your nationality is “American” Do not indicate a “passport number,” “date of issue,” “place of issue,” or “date of expiry” (this information will be filled in once your Peace Corps passport has been processed) Your occupation is “Peace Corps Volunteer” Use the address and phone number for Peace Corps headquarters for your business address (see template) Use the dates indicated on your Volunteer Assignment Description for “Date of Departure” under the Day-Month-Year format (i.e. 02-June-2011) Check the traveling by air box Write “No” where it asks if you are in possession of a return ticket and “N/A” where it asks for a ticket number Under “purpose of journey” check the “business” box and write “Peace Corps Volunteer” Use the names and addresses of references on the template 1
Use the Peace Corps Togo address provided on the template for your employer in Togo (“Peace Corps Togo, 48 Rue des Rossignols, Kodjoviakope, Lome”) Duration of stay is “27 months” Make sure that no blanks are left blank (except for your passport information). If something does not apply to you, simply write “N/A.”

Sign each copy legibly enough that your name can be read from your signature.
Mail your completed
Write down your tracking number before mailing off your applications so you know when they arrive.
