Peace Corps/Zambia Visa Requirements and Instructions PLEASE READ CAREFULLY: Incomplete or delayed submission of the visa application can result in you missing your program. To apply for your visa: Download and print the visa application at: http://www.zambiaembassy.org/sites/default/files/documents/Zambia_Visa_Application _updated_Dec-12-2012.pdf Please complete the application following the Zambia Visa Application example as a guide. Please complete and submit only the 3rd page of the pdf file.
Before printing, please confirm: Your surname is your last name. Write all dates in the DD/MM/YYYY format. For any blank that does not apply to you, write “N/A.” DO NOT leave anything blank. Nationality: “U.S. Citizen” Profession: “Peace Corps Volunteer” Business Telephone Number: “855-855-1961” Your permanent address is your home of record. Your permanent address and your present address can be the same. Type of visa requested: check the “volunteer” box Entry requested: check the “multiple” box Date of Entry into Zambia: Write the date two days after your staging event begins from your Invitation email. For example, if your staging date is June 11, 2017, write 13/06/2017. Length of Stay in Zambia: “27 months” Final Destination in Zambia: “Lusaka” Address in Zambia: “Peace Corps Zambia, 71A Kabulonga Road, Kabulonga, Lusaka” 1 3/27/17
Expected Date of Departure: 27 months after the date of entry. For example, if the entry date is 13/06/2017, your (estimated) departure date is 13/09/2019. Business persons to be visited in Zambia: “Leon Kayego, Country Director, 71A Kabulonga Road, Kabulonga, Lusaka” Addresses of Relatives or Friends: “N/A”
Once application is complete and reviewed, please: Print 2 visa forms Sign and date each visa form Paperclip 2 -2”x 2” photos passport photos to your applications (in addition to the 2 photos needed for your passport application.) This means that you will submit a total of and 4 photos-2 for each visa application.
*Please SIGN the back of EACH photo* Send your visa applications (and all supporting items) to the address provided using a mailing service (i.e. DHL, UPS, or Fed Ex). Please retain your tracking number.
