PaPOS Exit Survey:
If you volunteer to conduct the 2011 Exit survey, your LEA survey materials will be mailed directly to your district by March 15, 2011.

State Performance Plan (SPP) • Evaluate the State’s efforts to implement the requirements and purposes of IDEA • Describe how the State will improve such implementation – 6Year Plan – 20 indicators related to the 3 priorities – Annual Performance Report
1. Improve graduation rates for students with disabilities. 2. Decrease dropout rates for students with disabilities. 3. Ensure all students with disabilities participate in statewide or alternate assessments. 4. Reduce suspension and expulsion rates for students with disabilities. 5. Provide services for school-age students with disabilities in the least restrictive environment. 6. Provide preschool children with disabilities services in the least restrictive environment. 7. Improve cognitive and social outcomes for preschool children with disabilities. 8. Improve parent involvement in their child’s special education program. 9. Survey Population Indicator 14 Language Percent of youth who are no longer in secondary school, had IEPs in effect at the time they left school, and were: A. Enrolled in higher education within one year of leaving high school. B. Enrolled in higher education or competitively employed within one year of leaving high school. C. Enrolled higher education or in some other postsecondary education or training; or competitively employed or in some other employment within one year of leaving high 19 school.
Survey Population • All leavers were identified by the targeted LEA by examining the December 1, 2010 Federal Student Data Report (PennData). • All students between the ages of 17 and 21, with an IEP were considered possible “Leavers” for the 2010 11 School year.

Survey Population • Complete PaPOS Exit Survey with all student leavers who leave or are projected to leave between September, 2010 and June, 2011. – Leavers: include any student with an IEP who: • graduates with a high school diploma • dropouts • reaches maximum age OR • receives a GED
during the 20102011 school year.

Survey Population • Graduate with a high school diploma: includes any student with an IEP who graduated with a diploma between September 1, 2010 and June 30, 2011. • Dropouts: include any student with an IEP who dropped out of school between September 1, 2010 and June 30, 2011. • Reach maximum age: include any student with an IEP who reaches the age of 21 between September 1, 2010 and June 30, 2011. If a student leaves because he/she reaches maximum age, select this option rather than ‘Graduates with a high school diploma’. • Receive a GED: include any student with an IEP who receives a GED between September 1, 2010 and June 30, 2011. 22

Administrative Considerations
• Your exit survey packet will include both Prepopulated student surveys and blank surveys. The first two pages of instructions for these surveys differ. However, pages 35 are identical. • Blank Surveys must only be used for a student who is a leaver and who does not have a prepopulated form. • Please be certain that there is no existing pre populated form for the student before using a Blank Survey form.
Administrative Considerations • The PaPOS Exit Survey should be administered in an interview fashion by an individual familiar with the student being interviewed. • If a student’s disability limits his/her ability to respond accurately to the survey questions, an individual (e.g., family member, provider agency staff, etc.) can assist the student in answering the student survey questions.

Prepopulated Exit Survey Forms
PREPOPULATED FORMS – PAGES 12 • Information on students ages 17 – 21 was retrieved from the most recent PennData records. Data were placed into the exit surveys and separate surveys were generated for each potential leaver. • Leavers: include any student with an IEP who graduates with a high school diploma, dropouts, reaches maximum age or receives a GED during the 20102011 school year. • Students leaving under other circumstances
If a student is no longer attending your school, but does not meet any of the definitions above, place a note on the front of the survey indicating the reason that the student no longer attends school and the date of the change (e.g., moved to another district or out of state, no longer has an IEP, deceased, was a leaver prior to September, 2010, etc.)

Review and correct student demographics: • If the student is a leaver, review the information included from PennData. The data include the student’s last and first name (Questions 1 and 2) and other demographic information including District of Residence*, PA Secure ID, Date of Birth, Gender, Primary Special Education Disability, Ethnicity, (Questions 611). • Surveys in the Philadelphia SD include the name of the school along with the District. • If any of the demographic information is inaccurate, make corrections in the column entitled “Corrections.” If any information is missing from these fields (e.g., Prepopulated PA Secure ID is blank), record the information in the column entitled “Corrections.”

Find Section 3 Secondary Transition Activities and Services (the grid). Record one outcome for each question (1214). Be sure to respond to each question. Each has the option of ‘None’ or ‘Not Applicable’ which allows everyone to respond to every question.

Prepopulated and Blank Forms – Page 5 Contact information after leaving high school • Next year, the LEA is required to complete the PaPOS Post School Survey for each 201011 student leaver. Accurate contact information is critical for the LEA to follow up with these students. Contact information should be recorded for the student and a primary contact (the “best” or “emergency” contact), who must be someone other than the student. If any information is not applicable (e.g., the student or contact does not have a cell phone), enter N/A. Be sure to record the information fully (e.g., area codes with the phone numbers).
