2015 Volunteer Districts As per the PaPOS PennLink - All volunteer requests were required to be received by February 6, 2015 Volunteer LEAs for the 2015 PaPOS Exit Survey: • LEA survey materials will be mailed directly to your district by March 31, 2015. • Surveys will be color coded as a volunteer survey • Final submission of all completed 2015 volunteer exit survey forms must be mailed to PaTTAN Pittsburgh via UPS by May 29, 2015. 6

• Make a copy of the completed surveys for your records • Return the completed original colored surveys to PaTTAN Pittsburgh based on the submission target dates indicated on the Timeline and Procedures sheet. • Retain your UPS tracking receipt for your records. • To contact UPS for a direct pickup at your building call 1-800-PICK-UPS 27

Administrative Considerations • Your exit survey packet will include both pre-populated student surveys and blank surveys. The first two pages of instructions for these surveys differ. However, pages 3-5 are identical. • Blank Surveys must only be used for a student who is a leaver and who does not have a pre-populated form. • Please be certain that there is no existing prepopulated form for the student before using a Blank Survey form.

Administrative Considerations

The Exit Survey should be  administered in an  interview fashion by an  individual familiar with  the student being  interviewed.

If a student’s disability  limits his/her ability to  respond accurately to the  survey questions,  another individual can  assist the student in  answering the student  survey questions.

Review and Correct Student Demographics

• If the student is a leaver, review the information included from PennData. The data include the student’s last and first name (Questions 1 and 2) and other demographic information including District of Residence*, PA Secure ID, Date of Birth, Gender, Primary Special Education Disability, Ethnicity, (Questions 6-11). • Surveys in the Philadelphia SD include the name of the school along with the District. • If any of the demographic information is inaccurate, make corrections in RED in the column entitled “Corrections.” • If any of the demographic information is missing from these fields (e.g., Pre-populated PA Secure ID is blank), record the information 37 in the column entitled “Corrections.”

Pre-Populated and Blank Forms – Page 5 Contact information after leaving high school • Next year, the LEA is required to complete the PaPOS PostSchool Survey for each 2014-15 student leaver. Accurate contact information is critical for the LEA to follow up with these students. Contact information should be recorded for the student and a primary contact (the “best” or “emergency” contact), who must be someone other than the student. If any information is not applicable (e.g., the student or contact does not have a cell phone), enter N/A. Be sure to record the information fully (e.g., area codes with the phone numbers). 49
