PHASE II ESA OR LOCAL VOLUNTARY CLEANUP PROGRAM Upload SLLc2-2. The project team has uploaded an executive summary of the Phase II Environmental Site Assessment or local Voluntary Cleanup Program documenting the site contamination (Response Action Plan, Remedial Action Work Plan, etc.).

GOVERNMENT AGENCY OFFICIAL DESIGNATION Upload SLLc2-3. The project team has provided documentation from the local, state or federal government agency declaring the project site a brownfield (Resolution, Memorandum, Letter, listing from EPA Superfund website, etc.). Select one of the following: Remediation efforts have not yet been completed. The project team has completed remediation efforts of site contamination.
