All fields and uploads are required unless otherwise noted.
This static sample form has been modified for offline access. All sections of the form are visible. Sample forms are for reference only. Select one of the following: Stage 3A. No change since Stage 2 Stage 3B. Change since Stage 2 Stage 3C. Did not submit at Stage 2
STAGE 3A No change has been made since submittals in previous stages that would affect credit compliance. The project team achieved SLL Prerequisite 2 Stage 2 compliance.
STAGE 3B Provide updated versions of the items submitted at the previous stage.
STAGE 3C Upload SLLp2-1. The project team has uploaded written evidence that the state Natural Heritage Program (NHP) agency was consulted for the presence of species listed as threatened or endangered under the federal Endangered Species Act, the state's endangered species act, or species or ecological communities classified by NatureServe as GH, G1 or G2 on the project site. Upload SLLp2-2. The project team has uploaded written evidence that the state wildlife agency was consulted for the presence of species listed as threatened or endangered under the federal Endangered Species Act, the state's endangered species act, or species or ecological communities classified by NatureServe as GH, G1 or G2 on the project site. The results of the above inquiries to required agencies were: Conclusive Not conclusive

Select one of the following: A qualified biologist has been consulted and performed biological surveys per the credit requirement to determine if species / communities exist. Site conditions are such that the presence of species / communities is highly unlikely to occur (Complete required narrative and choose Option 1 below.) Upload SLLp2-3. The project team has uploaded a summary of the biological survey using accepted methodologies that meets the credit requirements.
Note: Do not upload entire document.
Describe why site conditions are such that the presence of species/communities is highly unlikely to occur.
Select one of the following: Option 1. Species and Communities Absent. The documentation provided for Upload SLLp2-1 and Upload SLLp2-2 conclusively demonstrates the absence of federal, state, or NatureServe (GH/G1/G2) classified species and/or communities OR the project's site conditions indicate that the presence of such species and/or communities is highly unlikely. Option 2. Species and Communities Present: Habitat Conservation Plan (HCP). The documentation provided demonstrates the presence of federal, state, or NatureServe (GH/G1/G2) classified species / communities and an approved HCP exists. Option 3. Species and Communities Present: Habitat Conservation Plan (HCP) Equivalent. The documentation provided demonstrates the presence of federal, state, or NatureServe (GH/G1/G2) classified species / communities; however, no HCP exists. The project team will prepare an HCP Equivalent.
SPECIES AND COMMUNITIES ABSENT The documentation provided for Upload SLLp2-1 and Upload SLL-p2-2 conclusively demonstrates the absence of federal, state, or NatureServe (GH/G1/G2) classified species and/ or communities OR the provided narrative thoroughly explains why species and communities are highly unlikely to occur on the site.
SPECIES AND COMMUNITIES PRESENT: HABITAT CONSERVATION PLAN (HCP) The project will comply with an existing regionally approved Habitat Conservation Plan (HCP) that covers each identified species or community.
LEED 2009 for Neighborhood Development SLL Prerequisite 2: Imperiled Species and Ecological Communities Conservation Form Version 4.0
Upload SLLp2-4. The project team has uploaded the cover page and an excerpt or executive summary from an HCP that demonstrates the project and its species / communities are covered by the HCP, and that the HCP has been approved by the U.S. Fish and Wildlife Service. Note: Do not upload entire document.
Describe how the project complies with the HCP.
SPECIES AND COMMUNITIES PRESENT: HABITAT CONSERVATION PLAN (HCP) EQUIVALENT Name of qualified individual or organization assisting with plan: Upload SLLp2-6. The project team has uploaded a summary of an HCP-equivalent conservation plan. The conservation plan includes the following elements: Identification and mapping of the geographic extent of the habitat and the appropriate buffer, not less than 100 ft. Protection of the identified habitat and buffer in perpetuity. Analysis of threats from development and a monitoring and management plan that eliminates or significantly reduces the threats. A portion of habitat and buffer identified cannot be protected in perpetuity. (Optional) Quantification of the impacts by acres or number of plants and/or animals affected, and onsite or off-site protection from development in perpetuity for an equal or greater amount have been provided.
ADDITIONAL DETAILS Special circumstances preclude documentation of prerequisite compliance with the submittal requirements outlined in this form.

SPECIAL CIRCUMSTANCES Describe the circumstances limiting the project team's ability to provide the submittals required in this form. Be sure to reference what additional documentation has been provided, if any. Non-standard documentation will be considered upon its merits.
Upload SLLp2-SC. The project team has uploaded any additional documentation that supports the claim to special circumstances. (Optional)
