All fields and uploads are required unless otherwise noted.

ALL PROJECTS This static sample form has been modified for offline access. All sections of the form are visible. Sample forms are for reference only. The project complies with all local, state, and federal regulations pertaining to wetland and water body conservation. The site includes exempted wetlands, water bodies, and/or buffer land. (Optional) Upload SLLp3-1. The project team has uploaded a project site base map which includes the project boundary and exempted wetlands, water bodies, and/or buffer land. The documentation provided for Upload SLLp3-1 demonstrates that the exempted wetlands, water bodies, and/or buffer land meet the following criteria (select all that apply): Previously developed land. Man-made water bodies (such as industrial mining pits, concrete-lined canals, or stormwater retention ponds) that lack natural edges and floors or native ecological communities in the water and along the edge. Man-made linear wetlands that result from the interruption of natural drainages by existing rights-of-way. Wetlands that were man-made incidentally and have been rated “poor” for all measured wetland functions. Upload SLLp3-2. The project team has uploaded the wetland quality assessment performed by a qualified biologist, using a method that is accepted by state or regional permitting agencies, to demonstrate the "poor" quality of wetland functions. Select one of the following: Option 1. Sites with no wetlands, water bodies, land within 50 feet of wetlands, or land within 100 feet of water bodies. Option 2. Sites with wetlands, water bodies, land within 50 feet of wetlands, or land within 100 feet of water bodies.

SITES WITHOUT WETLANDS, WATER BODIES, OR BUFFER LAND Upload L-2. The project team has uploaded a pre-construction project site base map with project boundary, natural features (existing topography, wetlands, water bodies, etc.), and built features (from PI form 3).

The documentation provided for Upload PI-10 demonstrates that the site does not include wetlands, water bodies, land within 50 feet of wetlands, or land within 100 feet of water bodies. SLL Prerequisite 3 Option 1 Stage 1 Compliance Documented:

Note: SLL Prerequisite 3 requirements are relevant to SLLc7: Site Design for Habitat or Wetland and Water Body Conservation Option 1.

SITES WITH WETLANDS, WATER BODIES, AND/OR BUFFER LAND Select one of the following: The site's preproject wetlands, water bodies, land within 50 feet of wetlands, and land within 100 feet of water bodies are not affected by new development unless it is minor improvements and/or previously developed land. The site earns at least 1 point under GIB Credit 8, Stormwater Management, and limits any impacts beyond minor improvements to less than the percentage of buffer land per SLL Prerequisite 3 requirements.

SITES WITH WETLANDS / WATER BODIES & MINOR / EXEMPTED IMPACTS The site's wetlands, water bodies, and/or buffer land are impacted by (select all that apply): New development on previously developed land (Optional) Only minor exempted improvements (Optional) Upload SLLp3-3. The project team has uploaded a project site base map which includes the project boundary, previously developed land, wetlands, water bodies, buffer land, and minor improvements.

Impacts on Previously Developed Land The documentation provided for Upload SLLp3-3 demonstrates that impacted wetlands, water bodies, and/or buffer land are located on previously developed areas of the site. Upload SLLp3-3. The project team has uploaded a project site base map which includes the project boundary, previously developed land, wetlands, water bodies, buffer land, and minor improvements.

Minor Exempted Improvements The documentation provided above demonstrates that wetlands, water bodies, and/or buffer land are only impacted by the following minor exempted improvements (select all that apply): Bicycle and pedestrian pathways no more than 12 feet wide, of which no more than 8 feet may be impervious. Activities to maintain or restore native natural communities and/or natural hydrology. One single-story structure not exceeding 500 sq.ft. per 300 linear feet of buffer, on average. Grade changes necessary to ensure public access. Clearings not exceeding 500 sq.ft. for tables, benches, and access for non-motorized recreational watercraft. Limit of one per 300 linear feet of buffer, on average. (Off-street parking is not eligible) Removal of hazardous trees; up to 75% of dead trees; trees under 6 inches diameter at breast height; trees under 40% condition rating; and up to 20% of trees over 6 inches diameter at breast height with condition rating 40% or higher. Brownfield remediation activities. Select one of the following: Set 1 Upload SLLp3-4. The project team has uploaded a commitment to prepare an ISAcertified arborist tree assessment.

Set 2 OR

Upload SLLp3-5. The project team has uploaded a copy of an ISA-certified arborist tree assessment.

No Minor / Exempted Impacts Upload SLLp3-6. The project team has uploaded a site base map which includes the project boundary, wetlands, water bodies, and buffer land. The documentation provided for Upload SLLp3-6 demonstrates that the wetlands, water bodies, and buffer land are not affected by any new development, minor improvements, or impacts beyond minor improvements. SLL Prerequisite 3 Option 2a Stage 1 Compliance Documented:

Note: SLL Prerequisite 3 requirements are relevant to SLLc7: Site Design for Habitat or Wetland and Water Body Conservation Option 1.

SITES WITH WETLANDS / WATER BODIES & MINOR / MAJOR IMPACTS The project team has completed GIB Credit 8: Stormwater Management Stage 1 and documented the following number of points: Minor improvements impact the wetlands, water bodies, and/or buffer land. (Optional) Upload SLLp3-7. The project team has uploaded a site base map which includes the project boundary, wetlands, water bodies, buffer land, minor improvements, and areas of water features impacted by development.
