CAMBODIA VISA INSTRUCTIONS READ CAREFULLY: Incomplete or delayed submission of your visa application can result in you missing your program! To Apply for Your Visa:  Download and print the visa application at:

Current Nationality: “U.S. Citizen” Present Occupation: “Peace Corps Volunteer” Work Place: “Peace Corps” Purpose of Visit: “Other – Peace Corps Volunteer” Point of Entry and Exit: “Phnom Penh” Address during the visit: “Peace Corps Cambodia” Organization, Persons to be visited: “Peace Corps Cambodia” Means of Transportation: “Air” Traveling on group tour: “No” Current Home Address is your Home of Record. Done in: where you are signing the form (city, state) Please leave the following blank: o Arrival and Departure Date in Cambodia o All passport information (number, place of issue, date of issue and expiration)

Guidance for Submitting Application:  Print 2 visa forms  Sign and date both visa forms.  Paperclip 2 - 2”x 2” photos (in addition to the 2 photos needed for your passport application) to your visa application. On the back of all

your photos, please print your name.

 Mail your passport application, visa application, and passport photos to the address provided . You need to use a mailing service to send us your travel documents (i.e. DHL, UPS, or Fed Ex). Please remember to retain your tracking number.
