Ball State University Basketball On-‐Campus Evaluation Athletics Compliance Office Policy & Procedure
NCAA Bylaw 13.11.2.1 On-‐Campus Evaluations -‐ Basketball. Under the NCAA rules and regulations, basketball prospective student-‐athletes may attend one (1) on-‐campus evaluation during an Official Visit or Unofficial Visit. Before a prospective student-‐athlete is permitted to participate in the on-‐campus evaluation, all of the following information must be submitted and approved by the Ball State Compliance Office no later than five (5) business days prior to arrival. • A copy of the prospective student-‐athletes’ most recently ended season schedule must be submitted, whether a high school/prep school senior, two-‐year transfer or four-‐year transfer. o This may be obtained from the team’s official site. • A completed Ball State Pre-‐Approval of Official Visit or Unofficial Visit Form. o These documents must also contain all required documentation for each form. • The date, time, duration and location of scheduled on-‐campus evaluation. o All OCEs must be conducted in a Ball State facility. • Documentation of a medical physical from a non-‐Ball State physician administered within twelve (12) months prior to the on-‐campus evaluation. o Ball State Athletics Training Staff will NOT provide physicals on site. • Documentation of a sickle cell solubility test or signature on the Sickle Cell Policy & Waiver form. o Ball State Athletics Training Staff will NOT secure a sickle cell solubility test on site. o A Ball State Sickle Cell Policy & Waiver form can be found on the Ball State Compliance or Athletics Training Website. • Signed Assumption of Risk and Release of Liability form. o ONLY basic first aid will be provided on site during the OCE. o The prospective student-‐athlete waives all further Ball State liability for risks and hazards. • Indication of whether equipment will be provided to the prospective student-‐athlete. o Equipment may be provided on an issuance and retrieval basis only. o Equipment Room Staff will confirm both issuance and retrieval. • Indication of whether current Ball State student-‐athletes will participate in the on-‐campus evaluation. o Ball State student-‐athletes may participate, provided such participation is counted toward the applicable hourly and weekly limitations on countable athletically related activities. APPLICATION PROCEDURE 1. Print and submit (paper, fax, or email) a “Student-‐Athlete OCE Packet” to the student-‐athlete. 2. Receive completed documentation from the student-‐athlete. 3. Submit Ball State OCE Evaluation Application, documentation (be sure all required documents are accounted for) and your official or unofficial visit form to the Ball State Compliance Office when all items have been secured. Upon receipt of the aforementioned documents, the Compliance Office will notify the appropriate basketball coaching staff of either approval or denial of the documents, and instruct on what must be completed if necessary. Once final approval has been provided to the appropriate basketball coaching staff, the Ball State Athletics Training Staff and Athletics Equipment Staff will be notified of the date and time of the proposed visit and OCE.
